I have received two Kadcyla treatments up to this point. I hope this works well for me for a long time. (Japanese to follow 日本語は最後に)

My estrogen + and HER2 + breast cancer metastasized to back and pelves bones as of October. I did radiation treatments to the back bone in November. I had been on hormone medication, Aromatase Inhibitor since October alongside a Lupron shot to put my ovaries in sleep. By mid-December, cancer was still spreading to chest bone.

My oncologist told me to be on Kadcyla, a HER2 treatment. It is an intravenous treatment, a mixture of a well known and well-working toward HER2 treatment called Herceptin and chemo elements are attached to the Herceptin. A very good visual is offered by a pharmaceutical company’s website about Kadcyla.

How does Kadcyla work.

Since it has a targeted element to it, the medicine is delivered to the cancer cell more directly rather than affecting the whole body. I probably do not have to lose hair, nor taste buds (5 years ago, it was worse with CTH chemotherapy treatment). However, it also carries some chemo element, relatively minor side effects are associated with it.

For now, I will be on Kadcyla for a long time, as long as it works. I am not told a finite number to count down.

So, here is what happened:

[Tuesday. December 31, 2019] The 1st Kadcyla Treatment

I went swimming in the morning to take full advantage of my well functioning body (my Instagram’s cancer friend inspired me.).

I cracked into a laugh so hard with a nurse, while I was about to get plucked on my finger for blood drawing. I just don’t like blood drawing. So, I told the nurse that she should tell me when she would do it so that I can take a big breath in and out. She told me to breathe in, and I was holding the breath. Instead of poking into my finger, nothing had happened. Then she asked if there was a cut on my finger. So I release the breath and laughed so hard because I was nervous with anticipation for a while but nothing had happened! Then we were both laughing and saying that she never had such a fun time at blood draw with a patient. Well, there was not cut and we did the second time and I was done.

My husband came along with me. At a chemo chair, I was feeling very nervous and the nurse told him to hold my hand. That was very nice. During the 90 minutes of infusion, all went well and we left around 5:30 pm. The nurse explained to us of precautious signs that might appear later for calling to seek advice to the hospital. We stopped by a grocery store on the way back to prepare us for the new year count down, and we came home. Traditionally, the New Year is like a Thanksgiving like in Japan, the biggest national holiday that everybody celebrates followed by a huge sale. I wanted to feel special.

We finished dinner, but I started to feel tired. I went to bed. I fall asleep quickly. I wake up shivering, feeling so cold. I had a fever of 102.8 F. After my husband called the hospital seeking to find out what to do, by 11:15 pm, we were told to go to the nearest emergency room. It was so hot and cold at the same time. I could not move well. I wanted to lie down.

The count down was with 2 nurses in the room about to draw (yet another) blood from me. “A happy new year” sure thing. They also did a flu test, gave me an anti-inflammatory shot, and a Tylenol kind of medicine. We came home at 2:30 AM.

[Wednesday-Thursday, January 1-2, 2020]

Fatigue, Nausea, severe Headache, and Vomiting. Flat in bed, crying so hard if I have to do this every 3 weeks. Horrible feeling. People were telling me that side effects were better than previous chemotherapy of 5 years ago, but this is similar. No appetite. Friends came to cook for us, thank you!

[Friday, January 3] My doctor’s office called me.

“Yoko, your flu test came back positive.”

What!? So, apparently, I had no coughing, nor running nose, but on top of the possible side effects that I have never experienced before, I had the flu. It was a relief, in a way. I don’t remember when I had flu the last time in my life. Being in academics, my family all gets flu shots every season. I don’t know what it is like to feel down with flu, nor with Kadcyla. Just great…

[Monday, January 6] I went out for the first time this year. I felt so thankful to be able to do that, it was a beautiful walk with Hoshi, our dog.

Over time, the symptoms were getting better. By Wednesday, January 8, I felt back myself and I went to the city for a photo shooting for an upcoming campaign, stay tuned!

I left for a vacation that I booked while my husband and I were waiting for hours before my first radiation in November. I found a great deal on flights and I had to book it on the spot to secure the great price of them. It was so nice to have something to look forward to in my future around that time. This trip changed my whole attitude. It was such a breakthrough.

Up until then, my life was taken over by cancer and I lost control of my emotions, life, and schedule. On this trip, I found myself again. I was away from my cancer appointments physically. I was making all the initiative on what to do, and where to go. Oh, I felt my life control back to me. I lost that sensation since the end of August. For 4 months, I was in a storm.

[Monday, 1/20/2020] 2nd Kadcyla, after my oncology dr. visit.

Allow me to start off with GREAT news. There was a thought I upheld to myself at the visit. Then my doctor told me, at the end of her examination on me, “don’t you think that your (initial tumor you found) tumor slightly smaller?” WOWOWOWOWOWow, yes, that is the thought I thought it might have been just my hopeful observation, so I did not say, but yes, I think it is smaller.

Dr. determined that my hormone treatment was not working on my tumors because my MRI in December was progressing only being on the hormone treatment. So she decided to let my ovaries be natural, no medical suppression and I became free of hormone medications which I was on something for the last 5 years. I felt so good about it. Oh boy, I don’t know where to start, but it is a relief to me. So we will be targeting our medical treatments for my cancer as more of HER2.

Kadcyla treatment. This time, it was only for 30 minutes of infusion. It takes more to prepare for it than the actual IV treatment. It all went smoothly. The only things to note were that a nurse for me that day was a Japanese, the first time! There was a free massage professional service done during the treatment. That was a major distraction.

[Tuesday, 1/21] Day 2 Down in bed the whole day. Fatigue, no appetite, headache, and nausea. Anti-nausea medication and Tylenol were helpful. I did not want to get up. I was so bored.

[Wednewday, 1/22] Day 3 after Kadcyla. I was up and walking again, but lighter fatigue, no appetite, lighter headache and gag-reflex. I took everything easy, and stayed home.

[Thursday, 1/23] Day 4 after Kadcyla. I went to the Fashion Institute of Technology for a ceremonial gathering for faculty, a convocation in the morning. I was so tired. I did not want to talk much. I stayed there in the morning and I head home to rest. By the evening, I attended a cancer support meeting that a local hospital provided near home. Fatigue, no appetite and gag-reflex, maybe minor headache as well.

[Friday, 1/24] Day 5 after Kadcyla. I went to NYC to get help in my mental situation. On my way, I was about to get hit by an NYC cab, totally the driver’s fault, I was fully aware of what was coming, but no energy to yell at him like an aggressive energetic New Yorker. Interesting findings of mental help for cancer patients, I hope to write a blog entry one day.

Overall, the Kadcyla recovery is like:

Day 1, Get a Zombie IV fluid.

Day 2, Being in a grave, getting ready to zombie around.

Day 3 and later, this zombie starts to move around and eventually becomes a human by Day 6.

OK… My new teaching starting with this new FUN medication every 3 weeks that having a Zombie Week. I prefer semi-annual Fashion Week. Wish me luck. So far, I have a full-timer’s teaching load, let’s see how it goes.

カドサイラ 抗体薬複合体治療 (副作用のほとんどなかった 分子標的で癌細胞にどんぴしゃりと攻撃する薬に 抗がん剤をくっつけたもの)をして、進めていくことになっている。この薬が長く効き続けてくれますように。

私の乳がんは、エストロゲンホルモンとHER2の両方の種類の乳がんで、背骨と骨盤に10月の段階で転移が確認されている。11月には、背骨に新しいタイプの放射線治療を施して、様子を見ている。同時に、10月からは、エストロゲンホルモンを抑制するために、卵巣の働きを注射で止め、ホルモンが癌にくっつくのを阻害する薬 アロマテーゼ阻害薬と言う飲み薬に変更され、ホルモン治療がどこまで効くか確認をしていた。12月に確認した際に、乳がんがさらに転移先が胸の骨にも広がっていったことから、ホルモン治療ではこの乳がんは止められないというので、今は、HER2で成長する乳がんだろうと踏んで、HER2を対象にした治療方法に切り替えをした。それが今回、大みそかから点滴治療が始まった、カドサイラ抗体薬複合体を点滴治療で入れるものに変えたという流れ。

カドサイラの仕組みは、このようになっている。中外製薬のウェブサイトから。(さすが、日本だ、漫画化している。上のアメリカのとは違いがあるな)


現段階では、私は3週間に一度、この薬が効くようであれば、効き続く限り投薬していくそうだ。終わりのない治療が始まる。前回の5年前の Stage 1トリプルポジティブのときの抗がん剤治療(CTH)は、6回限定でカウントダウンを楽しみにしていたのに、カウントダウンの意味がない戦いの始まり。

こんなことが起きた。

[2019年12月31日火曜日] 一度目のカドサイラ

朝、フルに動く体で楽しもうと、プールに行って泳ぎに行く。

午後、病院の点滴治療前の血液検査室で、担当の看護士さんと大爆笑する。血液検査が大の苦手の私は、あらかじめ針入れるときに言ってね、深呼吸するから と伝えておく。看護士さんが、息吸って、っていうから息吸って思いっきり息を吐きだしているのに、なかなか針が刺さらない。これ以上息が出ないよと言うのに 針が刺さってこない。すると、「ねぇねぇ、これ 傷口?」って 聞いてきて、結局針を刺さなかったようだ。こっちは緊張が一気にほぐれて、大爆笑。看護士さんに、こんなに笑った血液検査をしたのは初めてだわ。と言われる。可笑しかった。

ハズバンドが一緒に来てくれていた。抗がん剤治療の椅子の上で、看護士さんが不安げな私を見て、ハズバンドに左手を取ってもらうようにして 右手に点滴の針を入れる。すべてうまくいき、90分で終了。注意事項を説明してもらって、抗がん剤の味もあんまりしないし、気持ちも悪くないし よかった。正月の用意ができていなかったから、帰りに日本食の買い物をして家に帰って 夕食を食べた後8時過ぎに、なんだか気分も疲れたし、ダウン。ベッドに入る。

ぞわぞわと寒気で起きる。ハズバンドを呼びつけてたら、39度の熱。病院に連絡すると、専門担当医から連絡があって、近所の緊急室に行くようにとのこと。11時15分緊急室に行くけど、寒気で震えるし、横になっていた方が楽だ。

2人の看護士さんが、血液検査をするというので(そう、またあの嫌いな血液検査 また)用意している間に、熱でうなされながら カウントダウン。「ハッピーニューイヤー」 あーー はいはい、世の中そうですねぇ、ともう 別世界。インフルエンザのテストをし、炎症を抑える注射に、熱さましの薬をもらって午前2時半帰宅。

[水曜日 木曜日、1月1日と2日]

5年前の抗がん剤を彷彿させる だるさ、気持ち悪さ、頭は痛いし、嘔吐。ベッドから動けない。これが3週間に一度起きて、私はフルタイムの仕事をこなしていくつもりなのか。そして、永遠とこの薬をするのか。泣く。食欲無しだけど、日本食作れるお友達に来てもらって、おうどんやおかゆ用意してもらう、ありがとう~~~。

[金曜日 1月3日] 腫瘍内科の先生のオフィースから連絡で

「緊急室で受けてたインフルエンザの結果、あなた インフルエンザにかかっているわよ」

えーーー。と言うわけで、カドサイラの薬の副作用も未知の中、前回かかったインフルエンザは記憶がないほど インフルエンザがどういうものなのか体の記憶がないので、一体どこまでがインフルエンザで、どこまでが副作用かわからない状態に。お友達が 「泣きっ面に蜂」 と表現。まさしく。でも、これが3週間に一度ではないのかと思ったら、少し許せる気にもなってホッとした。咳も鼻水もでない、インフルエンザ予防接種を受けている、インフルエンザとは それだけだと、これまたどんなもんなんだろう。まあ あまり体験したくないから別にいいか。

[月曜日1月6日] 今年初、外を自分の足で歩く。あぁ、なんて素晴らしいことなんだろう。近所の犬の散歩なのに 涙が出てくる。

そんな具合で、徐々に良くなって、水曜日(カドサイラ8日後)には、マンハッタンで 癌患者さんたちのアメリカの非営利団体のプロジェクトで写真撮影にボランティア。また今度お楽しみに。

11月に放射線治療初日に 何時間もまたされた病室で、ハズバンド緊張して待っている間に、格安航空券情報が入ってきて、すぐに抑えないとなくなってしまうから、そんなタイミングで取った飛行機でカリブ、St. Martin セントマーチン島に行く予定にしていたので、次の治療の合間に、セントマーチンへ。気持ち悪さも多少残る中、そこで凄いことに気が付いてしまって 大収穫。失ってしまった、自分を見つけた という 詩的な表現で ぷーー っとしちゃうけど。9月から4か月、癌に振り回されて、追いついていくので心身精一杯。もう ついていけないことばかりで、普段の生活がままならない。そこに着て、病院から離れ、自分で取ったAirbnbで見つけた 個人で運営している宿に、フランス語で借りたレンタカー、まるで現地に住んでいるかのごとくに旅をするのは バックパッカーで経験をたくさん積んでいるからかもしれないけど。昔の得意分野で 自分がどんどんやりたいことを 自分のペースで進めることができる。自分主体の人生に再び出会うことになったのだ。これは、伝わらないかもしれないけど、非常に 非常に大きな発見で、失った自分を見つけた、という具合なわけだ。まさか、そういう発見ができるとは思わなかったから、少しの間その感覚にしがみつきたい。(インスタに写真いっぱい載せてる。@yoko_katz)

[1月20日 月曜日] 腫瘍内科の先生と会った後に、2度目のカドサイラ。

さて、ここで、ちょっと明るいニュースがある。先生に会ったときに、私の気のせいでは馬鹿らしいから私から言い出さなかったことがある。先生が全部の診察を終えた後、私の目を見てこういう。「ねぇ、あなたの見つけたこの腫瘍、今回初めて、小さくなっているように思わない?」 先生!!!! そうなんです。私がこの3週間、なんか ひょっとして 小さくなっている気がするけど、これは 私の気のせいかなと思っていたことを先生が口にしてくれた。うぁっほい、え、この薬効いてくれているのかな。あーーー、頑張りますから、効いてください。本当に小さくなっているなら、すごくうれしい。と言うことは、転移先も小さくなっているといいぞ。

今回、お医者さんは、私の2種類で栄養を得ている乳がんの治療をどちらの種類に対応したもので治療するかを悩んでいた。この3か月、私は 卵巣機能を止め、アロマテーゼ阻害薬を飲んでいる。これはホルモンの治療のみ。そして、12月のMRIで、乳がんに新たに転移先を確認してしまい、どうやらホルモンの治療は効果がないのかなと結論にたどり着くと同時に、今回 HER2の治療に移したとたんに、腫瘍が小さくなっているように感じると。その理由から、今回、私の治療から 一切ホルモンをターゲットにするのをやめてみることになりました。この5年間、日々の生活で 微妙な副作用がずっと5年続いていて不満を抱えていたので、そこから解放されるそうです。一つ 大きく息を吐く、ほっとした息を。

今回の治療は、初回の90分から30分に。すべてスムーズにいく。驚いたのが、全米でも有名な癌専門の病院で、初めて、日本で生まれ育ったという看護士さんに担当してもらいました!なんか、気分も変わったけど、それよりも、今まで全部英語でしかやってきていない 医療用語の日本語変換を知らなかったりするもんだから、二人でちゃんぽんではなしができていつもとペースが違ったこと。そして、無料のマッサージサービスの専門の人も来たら、あっという間の30分の点滴でした。

帰宅して、ご飯食べたら気分的に疲れて 寝る。夜8時。

[1月21日火曜日] カドサイラ2日目。ベッドから動けない。ひどい疲れ、頭痛、気持ち悪いし、食欲ない。さすがにインフルエンザがないので、熱がないだけまし。ひどさも前ほどではないけど。動けないのは、つまらない~~~。

[1月22日水曜日] カドサイラ3日目。あら、もう立って動ける。ただ、疲れているし、軽い頭痛、はかないけど、つわりのようにおぇってなる、気持ち悪いし、食欲なし。家の中で寝間着のまま、でもあまり集中できないので、なんとなく動いている感じ。

[1月23日木曜日] カドサイラ4日目。ちょっとしんどいなぁ、と思いつつも、お仕事の教授全体招集の学期始業式があるので、出かける。あーー、話す気もないし、午前中に出席して、午後には家に帰ってきて、昼寝。昨日からの同じような副作用が日に日に軽くなってくる。夜、近所の病院の乳がんのサポートグループに顔を出す。

[1月24日金曜日] カドサイラ5日目。午後に、マンハッタンで心理のヘルプを受けるのにいつもの病院に出かける。途中、青信号で渡っている私は 一歩下がったから 事故に合わなかったものの、左折してきたタクシーに危うく轢かれそうになる。なんだか、踏んだり蹴ったりなんですけど。NYでたまに見かける、タクシードライバーに おらぁぁ あぶねーじゃねーかー と叫んでいる人がいるけれど、そんなエネルギー一切なく、呆れて 目がまん丸になって、ありえない というジェスチャーで精いっぱい。タクシーのボンネットの上にタクシーの横から 手をついている私(それくらい近いところ)は タクシーの運転手を覗き込むので精一杯。あーー ごめんよごめんよー みたいに言ってた。あーー、なんか 死神さん近くにいるなー と思ってたら、Facebookのコメントで、「いつも守られているのよ!」との視点を投げかけてもらって救われる。そうだそうだ、私は いろいろなものに守られている。守られている。

さて、カドサイラだけでは、

初日、ゾンビになる点滴注射をする。

2日目、ゾンビになる、起き上がるのも大変。

3日目から 徐々に6日目にかけて、ゾンビが人間に戻ってくる。

と言うのがパターンなのかな。だとしたら、3週間に一度、ゾンビウィークがやってくる。半年に一度のファッションウィークのほうが良いな。今のところ、私はフルタイムで、最低限しないといけない1コマ3時間授業を週4コマこなしていく。プラス、大学のミーティング。そして、あと2年の間に、学術論文や論文発表などで功績を積んでいかないと、FITにフルタイムとして残れないかもしれない。普通の健康な体でもすごいプレッシャーの中こなしていくのに、私がここまで積み上げてきたものと、これから積み上げたいけど泣く泣くできないのではないかという不安と。一体どうなるだろう。1月27日月曜日から春学期開始。戦闘モードで日々の生活が流れる。遠くから近くから、応援頼んだ!
