Cancer stage changes overnight 一晩で変わる癌のステージ

I had been silent for the last 2 weeks about my new lump/cancer diagnosis. (日本語は英語の後に) Because of October 2, I was told that my cancer has spread to my bones. I don’t even know where to start. It took me 2 weeks for me to be able to write about this. (I am still processing. I will probably need some long term support from my friends, family, and communities. I am physically fine right now. I will probably need more help once treatments start, physically and mentally.)

Wednesday, October 2: I came home from my teaching. I quickly prepared and about to eat our dinner, I received a phone call from my oncologist. We were supposed to meet the following day. She told me the PET/CT showed breast cancer to the bones, 2 spots, very small. Something has run through my back to brain, an electric shock kind of thing I could feel, then my brain froze. She wants me to do a bone biopsy because, especially one spot, it is reachable. They will examine the specimen and determine my future treatments. They will identify whether they can remove these cancers or not. We canceled our meeting the following day and decided to meet with her in person after the biopsy report would be fully out there. The office staff would call me the following day to schedule the biopsy and we would meet the following week. Before she hung up, she told me “Yoko, you are not dying.”

I finished preparing the dinner table. The shock is gradually sinking in me. By the time of eating dinner, I completely lost appetite. I excused myself on a sofa. I started sobbing hard. I could not eat, and I could not sleep at all.

The next day, I went to attend a meeting at school, I had a hard time keeping myself not to go into a panic in a closed door. I cried so hard that day again. I did not face the facts, but I think I knew as soon as I heard of the spread, when they found metastasized to the bones, it made my 5-year stage 1 survivorship into stage 4 overnight.

Meanwhile, I could not get the biopsy appointment until 4 days later. The biopsy was scheduled further 10 days later. This made me go insane. I could only sleep for 1 or 2 hours at once. Some days, 1 hour, then the following day, I did not get sleepy at all. When I could sleep for 4 hours, it was a better one.

My oncologist office is not available for me to have any psychological aid. After numerous requests, I still do not hear from them. I called my breast surgeon. He prescribed me some medication to soothe my brain. He told me among other things, he said 10 to 20 years ago, with the technology then, people with bone metastasized patients had lived that long.

I lost 7 pounds in a week.

With the help of prescribed controlled substances, I could sleep for 9 hours. Oh, I felt so great. I am up for the bone biopsy.

The timeline that I have wrote so far….

  • 08/28/2019 Finding a new lump. Making a breast surgeon’s office appointment.
  • 09/06/2019 Dr. Appt: hand exam and ultrasound on the spot. Probably just fat necrosis. No problem.
  • 09/09/2019 The team radiologist did not confirm that was fat necrosis. Further test mammography test is scheduled.
  • 09/11/2019 Mammography. The radiologist could not confirm that it was fat necrosis…
  • 09/13/2019 Core Biopsy at the breast surgeon’s office.
  • 09/17/2019 Anxiously waiting for the results. Missed call. I could not speak to the doctor.
  • 09/18/2019 I called the dr and he called me back. It was cancer.

The timeline of what had happened since then.

  • 09/18 – 24 Anxiously wait for an insurance company to clear for MRI and PET/CT, on 24, finally scheduled for MRI
  • 09/27/2019 MRI. (I don’t like MRI. I asked for some sedatives. This was my 3rd MRI, in my life. I know what to do.)
  • 09/30/2019 PET/CT
  • 10/01/2019 Meeting with the breast surgeon. Without PET/CT scan result, MRI shows only the local recurrence. Great news!
  • 10/02/2019 My oncologist called me at night before dinner. She told me the PET/CT scan shows metastatic on the bones. 2 small spots.
  • 10/02-08, Trying to schedule for a bone biopsy. Finally, I have the schedule.
  • 10/15/2019 Meeting with a biopsy specialist
  • 10/16/2019 The bone biopsy.

2週間前の私の乳がんの話から、止まっている乳がんの話。10月2日に、ペットスキャンの結果、骨への転移が確認された。こうして書けるまでに2週間かかった。まあ、この際、ここに書いていった方がいい気がしたから 書いてみる。

10月1日火曜日。乳腺外科の先生が、MRIは例のしこりの一か所しか見つからなかったから、Pet Scanに何もないとして話を進める。結局、胸の筋肉ごとしこりを手術して取るから、また 乳房再建を新たにしないといけない。(つまり、最低全身麻酔2回)。まぁ、ほぼ みんなやったことあるのだから、様子がわかっている。自分が作ったトップス、私が一番最初に着るのかな。

10月2日水曜日。大学の授業から帰宅。急いでした夕食の準備が終わったころに腫瘍内科の先生から電話がかかる。木曜日に予約が入れてあるのに電話がかかる。どうやらペットスキャンに小さな癌が2か所骨に転移していることが確認されたということだ。体の後ろの方から脳みそのほうにかけて ビシィィ と電撃衝撃が走り抜ける。そして脳みそが固まる。今後、骨から細胞を取り出して、検査にかけて、骨から癌を手術で取り除くことができるものなのかを調べるそうだ。骨から取った細胞で、今後の治療が決まる。次の日の先生との予約をキャンセルし、骨細胞を取って調べる予約をするのに、係の者が木曜日に電話をくれることになる。先生は、電話を切る前に、「洋子、あなた、死んでくわけじゃないからね」と言う。

電話を置いて、食卓を並べる。だんだん先生との会話が私の中に落とし込まれる。食欲がなくなり、ソファーに移動し座る。ぼーっとするか、大泣きするかを繰り返す。食べられないし眠れない。次の日、大学のミーティングに出るけれど、狭い部屋の中にたくさんの教授が入って、息苦しい。パニック障害が出てきそうになるので、深呼吸をしてなんとかしのぐ。そのあたりの日は、すごくすごく泣きまくった。たぶん、頭の中ではなんとなく理解していたけれど、骨に転移したということは、やっぱり5年間ステージ1でサバイバー生活をして来たら、一晩でステージ4の最終ステージになる。その間、腫瘍内科の先生が言うような係の人からの連絡はないし、こちらから電話すること4日間。寝てないし、食べれない。せいぜい1時間・2時間の睡眠。4時間寝れると 良く寝れた方。それでも次の日 全然眠気に襲われない。4日電話しまくった後に、やっと骨の検査の日が決まると、その日から10日後だ。4日間で、こんなにボロボロなのに、あと10日間、そして結果がそれから出て 腫瘍内科の先生と今後のプランを立てるまで、一体何日間、この状態が続くのだろうか。もう限界。マンハッタンの道の真ん中で オフィースで タイミング悪く 係の人と電話をすると、大泣きする始末。1週間で3キロ減った。

あと10日間をしのぐのに、何か精神安定剤をお願いするものの、腫瘍内科の係の人は返事をくれない。乳腺外科の先生が精神安定剤を出してくれる。おどろおどろしい注意事項がいっぱい書いてある薬を出してもらう。乳腺外科の先生が、今から10年も20年も前の医療状態で、洋子の状況で10年 20年生きた人がいる。(そういう人は、ミラクル的だったのか、当時。そして たぶん、だから 今の医療はもっと長生きできるかもしれないと伝えてくれたのかな)

処方された薬を飲んだら、9時間寝た。久しぶりに寝た。すごい気持ちがよかった。

(そして、あれから私の感情は、ずっと日々変わり、この状況を受け入れないといけないと心で感情をいろいろすり合わせている。今の今は、体自体は痛くもかゆくもなく 良く動ける。たぶん、これから長く 体と心のヘルプがいる状況もあると思うから。今動ける体でいろいろやっておきたいと思っているので、今はめちゃくちゃ忙しい。お願いができるなら、今すぐこれができるよ!と声をかけてくれるのもありがたいけど、今後治療が始まってからこそ、ほそーーくながーーく 私をサポートしてもらえたらうれしいです。)

ここまで前にも書いたタイムライン

  • 08/28/2019 新たなしこりを見つける。一番早いアポを取る。
  • 09/06/2019 乳腺外科。触診と超音波検診。これは脂肪が壊死したやつだから 大丈夫。
  • 09/09/2019 チームのレントゲン技師がクリアサインを出さないので、マモグラフィーのアポ。
  • 09/11/2019 マモグラフィー。それでも クリアサインが出ないので、検査で細胞を取るアポ。
  • 09/13/2019 乳腺外科医が、針を入れてのコア検診。
  • 09/17/2019 結果待ち。授業中に電話が取れなかったら、一日話せずじまい。
  • 09/18/2019 私から電話をする。「洋子、申し訳ないけど、しこりは癌だった。」

それから起きたタイムライン

  • 09/18 – 24 保険会社からのGOサイン待ちで、24日、やっとMRIの予約が取れる。後日 PET CTも取れる。
  • 09/27/2019 MRI. 狭いところ好きじゃないし、もうMRIも人生で3度目になると、機械的に、リラックスできる薬を出してもらう。
  • 09/30/2019 PET/CT
  • 10/01/2019 MRIの結果だけで PETの結果が出てこない中 話を進める。どうやらMRIは一か所だけなので、手術で筋肉事取り除いて、再び再建手術をするから、2度全身麻酔の手術が待っているらしい。
  • 10/02/2019 夜に腫瘍内科の先生から電話を受ける。PET CTに転移が確認される。次の日の予約をキャンセル。
  • 10/02-08, 骨の検査をする検査日が決まらないので、何度も電話をする。
  • 10/15/2019 検査技師の専門医とアポ
  • 10/16/2019 骨の検査。
