Day19: Teaching Monday/Dr. Appointment

Day 19 after my 5th chemotherapy. I always learn something when I visit doctor’s office.

My next and the last chemotherapy (Hooray!) is coming up this Wednesday. I explained about my leg situation once more as we always review how I reacted for each chemo, even though some of the side effects have been the same as before. I also asked what kind of dose I would be getting for the next treatment.

I am getting the half dose, the same as my 5th treatment. New discovery for me was that the constant tingling sensation of my toes was actually from nerve system, and not from being cold. The doctor was afraid of permanent damage on my nerve system therefore, I am getting the half dose this time as well. (I always forget to ask something. This time, I forgot to discuss about my occasional puffy feet.)

As I am getting the last chemo treatment, some other topics came up, reconstruction and hormone medication. I should contact my plastic surgeon for my reconstruction process. I now have a temporary material called expander which stretch my skin and muscle as some of my breast skin was also trimmed off with cancer tissue. Before the final exchange of the expander to silicon, the expander needs to be gradually inflated more with saline water to stretch my skin even further more. This process takes time. Any major surgery has to wait at least (my memory is not clear but) 4 weeks after the last chemotherapy. Timing wise, it is getting the time to contact the plastic surgeon to discuss about my next plan. After the chemo, I seem to be starting daily hormone medication for at least 5 years. The recent study shows 10 years is better than 5 years, but I can wait that decision 5 years from now.

I was talking to a nurse before the doctor came to the room I was in. She congratulated me on the last chemo, while expressing how I survived so far with a very tough kind of chemotherapy. I know it was hard on me, but I was curious to find out how hard it was relative to to other kinds of breast cancer chemo treatments. I asked her to scale my regimen based on harshness in the scale of 1 to 10, as 10 to be the hardest. She would say it was 8 to 9 range. There is another kind that she knows that is hard. OK, officially, I would say it has been tough!!!

Oh, this time, weight is the heaviest in the past 3 years! Who knows how I will be after my last chemo…

[Day 19] tingling on my toes. My husband pulled me uphill after the work day. In subway in the city, occasionally, I was worried whether I was getting panic attacks.

5回目抗がん剤治療後19日目。お医者さんとお話しすると、毎回いろいろ発見がある。(興味深いけれども、自分のことと成ると 真実は時に怖い。)

いよいよ、今度の水曜日が私の最後の抗がん剤治療。お医者さんに、私の脚の痺れと状態を説明する。これは、毎回抗がん剤治療前に、同じことであっても、この3週間でどんな様子だったかを説明してくれと求められていることである。

今回も、私は半分の量の抗がん剤治療をするようである。前回と同じ量。つま先の痺れに関しては、どうやら寒さからくるのではなく、神経からきているようである。だから お風呂でどれだけ温めても、つま先のぴりぴり感が取れないのだろう。お医者さんは、神経が一生ダメージを受けないためにも、半分の量でやめておくと決定をしている。(脚のむくみについて相談するつもりだったのに、聞き忘れる。3週間のうちに、聞きたいことがでてくるのに、いつも聞き忘れたり、看護婦さんに話してメモを取ってもらっていても、看護婦さんが話題を先生に振り忘れて、私も質問はすでに伝えたつもりになっていて、聞き損ねて帰宅することもしばしばである。)

最後の治療が終わるにあたって、いくつかの新しい展開があった。再建の話と、ホルモンの薬治療のことである。左胸全摘出しているので、その再建でそろそろ、整形の先生に連絡を取って話を進めるのがいいとのこと。最終的に、シリコンを入れるのだが、今は仮のエクスパンダーというものが入っている。摘出時にとってしまう皮膚や、シリコンは胸の筋肉の下に入るので、筋肉などの組織を膨らませていく仮のものである。長い時期をかけて塩水を少しずつエクスパンダーに注入して大きくする。そして、最終的に皮膚などが伸びたところで、シリコンを入れる。抗がん剤治療終了後、4週間は最低あけないと手術が出来ないので、今から連絡して話を進められるそうである。もうひとつは、最後の治療が終わったら、毎日5年間薬を飲み続けるホルモンの治療が始まるらしい。研究によっては、10年飲むほうが良い結果が出ているのだが、5年飲むか10年飲むかは、5年後の最新の研究を見てから決めればよいので、今は考えずに飲むだけだな。

いつも、先生が部屋に来る前に看護師さんとのやり取りがある。いつもの顔見知りの看護師さんが入ってくるなり、私の治療が最後になるのを喜んでくれた。そして、強い薬の治療を良く耐えてやってきたとほめてくれる。確かにきつい治療だったのは身を持ってわかっている。どの程度きついものかと言うのが知りたくなった。乳がんの抗がん剤治療の中でも、どの程度きつい治療だったのか。1から10の単位で、10に近づくほどきつい治療だとすると、私がしているCTH+Pという治療はどの程度強いの?看護師さんが言うには、8から9の強度だと思うと。もうひとつ もっときつい治療があると言っていた。ふむふむ、なるほど!さて、ここで 大きな声で言おう!この抗がん剤治療は きつい!!!

ところで、止まらぬ食欲のおかげで、私の体重は ここ3年でもっとも重い体重と成っていた。恐ろしくて、体重計に乗らずに過ごしていた。さて、最後の抗がん剤治療が終わる頃にはどうなってしまっているのか。

[Day 19] つま先がぴりぴり。ハズバンドが、駅から家までの坂道を引っ張って歩いてくれる。マンハッタンの地下鉄では、いつパニック障害がでてくるのかと心配して乗っていた。心配しているだけでも、恐怖心がある。

    いよいよ、最後の抗がん剤治療だね、このブログを読ませてもらって。抗がん剤治療の辛さがホント伝わったよ、俺が感じるならヨウコの辛さは10のレベルだよ。絶対いい流れで来てると、俺は信じてるからあと一回も必ず上手く行くはず!ヨウコ、ラストアタックだ!ガンバレ!

      ありがとう。
      うん、いよいよ 最後だ。
      癌になって 初めて知ることが多かったけれど、同じ癌でも抗がん剤の種類もたくさんあって、ものによって 副作用に違いがあることも新しく知ったことのひとつ。人によって 体のリアクションも違うんだって。
      だから、「あの人は、抗がん剤を打って来ても、すぐに庭仕事してたわよ。」
      「抗がん剤に負けずにランニングを続けたんだって」
      と話を聞いて、私も よし!若いんだし、負けずにバリバリがんばろう。と思ったら へこたれてた。
      最後になって、客観的に観察していても、辛い種類のものだと知って、
      あー やっぱりそうだよネェ、庭仕事もランニングどころか、坂道すら登りきれない自分に 「がんばったねぇ」と 自分で言いたいよ!
      気持のアップダウンと付き合うのが 今は 辛いかな。
      目をつぶっていれば、時が過ぎて 身体が楽になっていくから、何とか 気を紛らわせて時が過ぎていくのを待てたらいい。
